Antenatal simply means before birth, and the term is widely used in Singapore, the UK, Australia, and most Commonwealth countries in place of the American term prenatal. Antenatal care includes the full range of support from your first booking appointment through to delivery, covering blood tests, scans, group B strep screening, and birth preparation classes. You may also hear antenatal used in phrases like antenatal depression, which describes low mood or anxiety during pregnancy, not just after. Knowing the term helps you understand clinic letters, hospital paperwork, and health resources from KKH or NUH without confusion.

Amniocentesis is a diagnostic test usually performed between 15 and 20 weeks of pregnancy. Using continuous ultrasound guidance, a thin needle is passed through the abdomen into the uterus to withdraw a small sample of amniotic fluid, which contains cells shed by the baby. These cells are cultured and analysed to detect chromosomal conditions such as Down syndrome, as well as neural tube defects and some inherited genetic disorders. Results can take one to two weeks, though rapid testing for common trisomies may be available sooner. The procedure carries a small miscarriage risk of around 0.1 to 0.5% when performed by an experienced specialist.

The anatomy scan is a detailed ultrasound performed between 18 and 22 weeks of pregnancy. A sonographer carefully examines your baby's brain, heart, spine, kidneys, limbs, and facial features to check that everything is developing normally. The placenta position and amniotic fluid levels are also assessed. If a low-lying placenta is spotted, a follow-up scan later in pregnancy is usually recommended. The scan takes around 30 to 45 minutes. While it detects many structural concerns, it cannot rule out every condition, so results should always be discussed with your obstetrician.

Abdominal circumference (AC) is the measurement taken around your baby's belly during an ultrasound scan. It is one of the key measurements used in your second and third trimester anomaly and growth scans. AC reflects the size of your baby's liver and the amount of fat stored under the skin, making it a good indicator of overall nutrition and growth. When combined with head circumference, femur length, and biparietal diameter, it helps your doctor estimate your baby's weight and check whether growth is on track. A reading that is too small or too large may prompt closer monitoring or further investigation.

An antibody screen, sometimes called an indirect Coombs test, is a blood test done early in pregnancy to check whether your blood contains antibodies that could harm your baby. These antibodies can develop if you have received a blood transfusion in the past, had a previous pregnancy, or are Rh negative and were exposed to Rh-positive blood. If antibodies are found, your doctor will monitor you and your baby more closely throughout pregnancy, as certain antibodies can cross the placenta and destroy the baby's red blood cells. The test is painless and forms part of routine antenatal bloodwork at your booking visit.

The amniotic sac is a thin but tough pair of membranes, the amnion and chorion, that form a sealed, fluid-filled chamber around your growing baby inside the uterus. It protects your baby from bumps and pressure, helps maintain a stable temperature, and gives your baby room to move and develop normally. The sac usually ruptures at the start of labour, which is what people mean by "waters breaking." In some cases, doctors may rupture it artificially to speed up labour, a procedure called an amniotomy. A healthy, intact sac is essential for a safe pregnancy.

Amniotic fluid is the clear, yellowish liquid that fills the amniotic sac and cushions your baby throughout pregnancy. In early pregnancy it is mostly water from your body, but from around 20 weeks your baby's urine becomes the main source. The fluid lets your baby move freely, which is important for healthy muscle and bone development. Your baby also swallows and breathes the fluid, which helps the lungs and digestive system mature. Fluid levels naturally peak around 34 to 36 weeks and then gradually decrease toward your due date. Too much or too little fluid can be a sign that something needs attention.

The amniotic fluid index, or AFI, is a measurement your doctor or sonographer takes during an ultrasound to estimate how much fluid is around your baby. The abdomen is divided into four sections and the deepest pool of fluid in each is measured in centimetres. The four numbers are added together to give the AFI score. A normal AFI is generally between 8 and 24 centimetres. An AFI below 5 to 8 may suggest oligohydramnios, while a score above 24 may indicate polyhydramnios. Both conditions can affect your baby's wellbeing and may need closer monitoring or treatment.

An anterior placenta is simply one that has attached to the front wall of the uterus, the side closest to your belly button. This is a normal finding and does not harm your baby. However, because the placenta acts as a cushion between your baby and your abdominal wall, you may feel your baby's kicks and movements later than expected, or they may feel more muffled and low. Kicks are often felt more strongly on the sides or lower abdomen. An anterior placenta can also make it slightly harder to hear the heartbeat clearly with a home doppler and may affect how amniocentesis is performed.

Active labor begins once the cervix reaches around 6 centimetres dilated and continues until it is fully open at 10 centimetres. Contractions become longer, stronger, and more regular, typically coming every 3 to 5 minutes and lasting 45 to 60 seconds. Most women find active labor significantly more intense than the latent phase and need focused breathing or pain relief to manage. This is the stage when most hospitals encourage you to come in. Cervical progress tends to be more steady during active labor, though the pace still varies from person to person.

Augmentation of labor means helping a labor that has already begun but is moving too slowly or has stalled. Your doctor or midwife may suggest augmentation if contractions are too weak, too far apart, or have stopped progressing after your cervix started dilating. The most common method is an oxytocin drip (Syntocinon) given through an IV to strengthen and regularize contractions. Another approach is breaking the waters (amniotomy) to encourage stronger contractions. Augmentation is used to protect both mother and baby when slow labor raises concerns about infection, fetal distress, or exhaustion.

Amniotomy is the deliberate breaking of the amniotic sac by a doctor or midwife using a small, thin hook called an amnihook. It is also called artificial rupture of membranes, or AROM. The procedure takes only seconds and is usually painless, though you will feel a warm gush of fluid. It is done to start or speed up labor, to check amniotic fluid color for signs of meconium, or to place an internal fetal monitor. Amniotomy is only performed after you are already in labor or being induced, and it commits your body to delivering within a certain time frame to reduce infection risk.

Assisted vaginal birth is when a doctor or midwife uses an instrument, either forceps or a vacuum cup, to help guide the baby out during the pushing stage of labor. This may be needed when labor is not progressing, when the mother is exhausted and cannot push effectively, or when the baby shows signs of distress and needs to be born quickly. It is performed under local or regional anesthesia to reduce pain. While it sounds alarming, assisted birth is a well-practiced skill that can prevent the need for emergency C-section when used at the right moment.

The Apgar score is a quick health check given to newborns at 1 minute and again at 5 minutes after birth. A midwife or doctor assesses five signs: skin colour, heart rate, reflex response, muscle tone, and breathing effort. Each sign is scored from 0 to 2, giving a total out of 10. A score of 7 or above is considered healthy. A lower score means the baby may need extra support, such as warming, suctioning, or oxygen. The score helps the care team decide quickly if a baby needs immediate medical attention.

Afterpains are cramping sensations caused by your uterus contracting after birth as part of the involution process. They feel similar to menstrual cramps or mild labour contractions and tend to be stronger with each subsequent pregnancy. Breastfeeding can intensify afterpains because nursing triggers oxytocin, which causes the uterus to contract more firmly. Most afterpains are strongest in the first two to three days postpartum and gradually ease off. Paracetamol or ibuprofen (if not contraindicated) can help manage the discomfort. Staying hydrated and emptying your bladder regularly also helps the uterus contract more efficiently and reduces pain duration.

Basal body temperature (BBT) is your lowest resting body temperature, measured first thing in the morning before you get up, talk, or eat. After ovulation, the hormone progesterone causes BBT to rise by about 0.2 to 0.5 degrees Celsius and stay elevated until your next period. By charting these daily readings over several cycles, you can identify a pattern that reveals when you ovulate. This helps you time intercourse in future cycles, since the rise confirms ovulation has already occurred. A consistent BBT chart can also be shared with your gynaecologist if you are investigating fertility concerns.

Beta hCG (human chorionic gonadotropin) is a hormone produced by the placenta after a fertilised egg implants in the uterus. A blood test measures the exact level in mIU/mL, giving more precise information than a urine pregnancy test. In early pregnancy, doctors track two readings taken 48 hours apart. A healthy pregnancy typically sees levels double every two days. Very low or slowly rising numbers can signal a possible miscarriage or ectopic pregnancy, while unusually high levels may indicate twins or a molar pregnancy. Your doctor uses trends over time, not a single reading, to assess how your pregnancy is progressing.

A birth partner is the person you choose to be by your side during labor and birth, offering continuous support and advocacy throughout the process. This can be your spouse, a parent, a close friend, or a hired doula. A good birth partner helps you stay calm between contractions, communicates your preferences to the medical team, fetches water, and simply holds your hand when words are not enough. Being well-prepared together matters: attending antenatal classes as a pair, discussing your birth preferences in advance, and agreeing on when to head to the hospital are all steps that make a real difference.

A birth plan is a written summary or conversation that outlines your preferences for labor, delivery, and immediate newborn care. It may cover pain relief options, preferred delivery positions, who cuts the cord, delayed cord clamping, skin-to-skin contact, and feeding intentions. While no one can guarantee that birth will follow a script, sharing your preferences helps the care team understand what matters most to you, and opens up an important conversation about what is realistically possible. Keep it concise, ideally one page, and frame preferences as wishes rather than demands so the team can work with you flexibly as the situation evolves.

Biparietal diameter (BPD) is an ultrasound measurement taken across the widest part of your baby's skull, from one side of the head to the other. It is one of the key measurements used to estimate gestational age in early pregnancy and to monitor fetal growth as pregnancy progresses. BPD is measured in millimetres and plotted against standard growth charts. A result that falls outside the normal range does not automatically indicate a problem, as head shape, fetal position, and ethnic variation all affect the reading. Your obstetrician will always interpret BPD alongside other measurements before drawing any conclusions.

Blood pressure is measured at every antenatal visit because it is one of the earliest warning signs of complications such as gestational hypertension and pre-eclampsia. A normal reading is usually below 130/85 mmHg in pregnancy. Readings consistently at or above 140/90 mmHg require further assessment, including urine tests for protein and blood tests. Pre-eclampsia can develop rapidly and is dangerous for both mother and baby if left untreated. Even if you feel fine, a high reading should never be ignored. Your care team will decide whether monitoring at home, medication, or admission is needed based on the full clinical picture.

Braxton Hicks contractions are occasional, painless tightenings of the uterus that can start as early as the second trimester and become more noticeable in the third. They are the uterus practising for labour, but unlike true labour contractions, they are irregular, do not increase in frequency or intensity, and usually ease off when you change position, drink water, or rest. They feel like a squeezing or hardening across the abdomen that lasts 30 to 60 seconds. If contractions become regular, grow stronger, or come with back pain or fluid leakage, contact your doctor or head to the delivery suite.

Bloody show is a small amount of blood-tinged or pink mucus discharge that appears as the cervix begins to dilate and efface in preparation for labour. It happens because tiny blood vessels in the cervix break as it stretches and softens. Bloody show can occur hours or even a few days before active labour begins, and it is considered a normal sign that your body is progressing. It often appears after a cervical examination by your doctor or midwife. The amount is usually small, roughly a teaspoon or less, and is mixed with mucus rather than appearing as fresh, flowing blood.

A birth pool is a large, inflatable or permanent tub filled with warm water that you can use during labor or for the actual birth. Warm water immersion helps ease contraction pain by relaxing your muscles and reducing pressure on your body. It can lower the need for pain medication and help you feel more in control. You can enter the pool in active labor and exit at any time. The water temperature is kept around 37 degrees Celsius to keep you comfortable and prevent your baby from overheating. A midwife monitors both of you throughout.

Breech presentation means your baby is positioned with their bottom, feet, or knees pointing toward your cervix instead of their head. This occurs in about 3 to 4 percent of full-term pregnancies. Most babies turn head-down on their own before 36 weeks, so a breech position found at an earlier scan is not usually a cause for alarm. However, if your baby remains breech by 36 weeks, your obstetrician will discuss your options, which may include a procedure called external cephalic version to turn the baby, or planning a caesarean section for a safer delivery.

Back labor refers to intense, persistent pain felt in the lower back during contractions, and sometimes between them. It is most common when the baby is in the occiput posterior (OP) position, meaning the baby's skull presses against your spine and tailbone. The sensation is often described as deep, aching pressure that does not fully ease between contractions. Comfort measures include counter-pressure applied firmly to the lower back by a partner or doula, warm compresses, changing positions such as hands-and-knees, or hydrotherapy if available. An epidural is also effective at relieving back labor pain.

Bilirubin is a yellow pigment produced when the body breaks down old red blood cells. Newborns have a higher turnover of red blood cells than adults, and their livers are not yet fully efficient at processing and excreting bilirubin, so levels can rise quickly after birth. Bilirubin is measured via a blood test or a non-invasive skin device called a transcutaneous bilirubinometer. The result is plotted on a chart that takes into account the baby's age in hours, gestational age, and risk factors. Doctors use this to decide whether the baby needs phototherapy or simply more frequent feeding and monitoring.

Baby blues are a very common emotional adjustment that affects up to 80 percent of new mothers in the first one to two weeks after birth. You may feel weepy, irritable, anxious, or emotionally fragile even when things are going well. These feelings are caused by the dramatic drop in oestrogen and progesterone that happens right after delivery, combined with sleep deprivation and the enormous life change of becoming a parent. Baby blues are temporary and usually resolve on their own by day 10 to 14. Rest, practical help from family, honest conversations, and gentle reassurance are the most useful supports during this brief window.

Cervical mucus is a fluid produced by glands in the cervix that changes in texture and volume throughout your menstrual cycle, driven by oestrogen and progesterone. In the days before ovulation, mucus becomes clear, slippery, and stretchy, similar to raw egg white, which allows sperm to survive and travel toward the egg more easily. After ovulation it becomes thicker and cloudy, forming a barrier. Tracking these changes each day, alongside other fertility signs like BBT, gives you a clearer picture of your fertile window and can improve your chances of conceiving.

A chemical pregnancy is an early pregnancy loss that happens shortly after implantation, usually within the first five weeks. The embryo produces enough hCG to trigger a positive pregnancy test, but it stops developing before a heartbeat or gestational sac can be seen on ultrasound. Many women experience a chemical pregnancy without realising it, as the loss often looks like a slightly late or heavier period. While it can be emotionally painful, especially after fertility treatment, a chemical pregnancy does not usually indicate a fertility problem. Most women go on to have a healthy pregnancy in subsequent cycles.

Crown-rump length (CRL) is the measurement taken on ultrasound from the top of the embryo's or fetus's head (crown) to the bottom of its bottom (rump), deliberately excluding the legs because they are curled up. This measurement is the most accurate way to estimate gestational age in the first trimester, typically between 6 and 14 weeks. A small change in CRL corresponds to a precise number of days of development, making it a reliable tool for confirming your due date early on. After 14 weeks, other measurements such as head circumference and femur length become more useful for tracking growth.

Cell-free DNA (cfDNA) screening is another name for NIPT. During pregnancy, small fragments of DNA from the placenta enter your bloodstream. A blood sample is taken from you and analysed in a laboratory to look at these fragments for signs of chromosomal abnormalities. The test can be done from 10 weeks and is considered far more accurate than older combined first trimester screening for conditions like Down syndrome. Results are usually returned within one to two weeks. Because it looks at placental DNA rather than the baby's DNA directly, rare false positives and false negatives can occur.

Chorionic villus sampling (CVS) is a diagnostic procedure performed between 10 and 13 weeks of pregnancy. A small sample of placental tissue, called chorionic villi, is taken either through the abdomen or the cervix using a fine needle or tube guided by ultrasound. The tissue shares the baby's genetic make-up, so it can be tested to diagnose chromosomal conditions like Down syndrome, as well as inherited genetic disorders such as thalassaemia. CVS provides a definitive yes or no answer, unlike screening tests. The procedure carries a small risk of miscarriage, generally estimated at around 0.5 to 1%, and should be performed by a trained specialist.

Constipation means having fewer than three bowel movements a week, or passing hard, dry stools that are difficult or painful to move. During pregnancy, the hormone progesterone relaxes the bowel muscles, slowing digestion. Iron supplements, which many Singapore mums take throughout pregnancy, make this worse. As your uterus grows it also puts pressure on your bowel. To ease constipation, increase fiber through fruits, vegetables, and wholegrain bread, drink at least eight glasses of water daily, and take short walks regularly. If diet changes are not enough, your doctor or midwife can recommend a safe stool softener. Straining too hard can lead to hemorrhoids, so act early.

The cervix is the narrow, lower end of the uterus that connects it to the vagina. During pregnancy it stays firm and closed, sealed by a mucus plug that protects the baby from infection. As labor approaches, the cervix softens (ripens), shortens (effaces), and gradually opens (dilates) to allow the baby to pass through. Your doctor or midwife may perform an internal examination to assess how dilated and effaced your cervix is during late pregnancy or active labor. Dilation is measured in centimeters, with 10 cm being fully open and ready for pushing.

Cervical length refers to the measurement of your cervix taken via transvaginal ultrasound, usually between 18 and 24 weeks of pregnancy. A longer cervix generally indicates a lower risk of early labor. A typical measurement in mid-pregnancy is around 35 to 48 mm. Your doctor may recommend this scan if you have had a previous preterm birth, a prior cone biopsy, or are carrying twins. The result is always interpreted alongside your full clinical history, because a single number does not tell the whole story on its own.

Cerclage is a minor surgical procedure where a doctor places a strong stitch around the cervix to keep it closed during pregnancy. It is recommended for women with cervical incompetence, meaning the cervix begins to shorten or open too early, which can lead to miscarriage or premature birth. The stitch is usually placed between weeks 12 and 14, and removed around week 36 to 37 when it is safe for labour to begin. Your doctor will monitor cervical length with ultrasound scans to decide if cerclage is the right option for you.

A contraction is the rhythmic tightening and relaxing of the uterine muscle that helps open the cervix and push your baby down during labor. Real labor contractions become progressively longer, stronger, and closer together over time. Early contractions may feel like strong period cramps or a tightening across the abdomen and back. You can time them from the start of one to the start of the next. Most hospitals advise heading in when contractions are 5 minutes apart, lasting about 1 minute, for at least 1 hour. Braxton Hicks contractions, sometimes called practice contractions, are irregular and usually painless, and they do not cause cervical change.

Crowning happens during the pushing stage of labor when the widest part of your baby's head becomes visible at the vaginal opening and stays visible between contractions, rather than slipping back. This is a sign that birth is very close. You may feel an intense burning or stretching sensation, sometimes called the "ring of fire." Your midwife or doctor will guide you to slow your pushing at this point to allow tissues to stretch gradually, which can reduce tearing. Warm compresses, controlled breathing, and gentle perineal support from your care team all help during crowning.

Cardiotocography, or CTG, is a test that records two things at the same time: your baby's heart rate and your uterine contractions. Two sensors are placed on your abdomen and held with elastic belts. The machine prints a paper trace or displays readings on a screen. Doctors and midwives look for patterns that show the baby is getting enough oxygen. A normal CTG shows a baseline heart rate of 110 to 160 beats per minute, regular small variations, accelerations with movement, and no concerning dips called decelerations. Abnormal patterns may prompt further assessment or a change in your care plan.

Cervical ripening refers to the process of softening, thinning, and opening the cervix to prepare it for labor. Before a doctor can induce labor, the cervix often needs to be made more favorable. Methods include prostaglandin gels or tablets placed near the cervix, a small balloon catheter inserted to gently stretch the opening, or oral medications like misoprostol. The approach depends on how far along the cervix has already changed, the baby's position, and the mother's health history. Ripening usually happens in the hospital over several hours, and the care team monitors both mother and baby throughout the process.

A cesarean birth is a surgical procedure where your doctor makes an incision through your abdomen and uterus to deliver your baby. It usually takes around 45 minutes and is performed under regional anesthesia, so you are awake but feel no pain below your waist. A cesarean may be planned ahead of time or happen urgently during labor. Recovery takes longer than a vaginal birth, typically four to six weeks, and you will have a small scar just above your pubic hairline. Most people go home within three to four days and can breastfeed and bond with their baby right away.

Cord blood is the blood left in your baby's umbilical cord and placenta immediately after birth. It is rich in haematopoietic stem cells, which can develop into different types of blood cells. These stem cells are used in treatments for certain cancers, blood disorders, and immune diseases. Parents can choose to donate cord blood for free to a public bank, where it may help other patients, or pay to store it in a private cord blood bank for their family's potential future use. The collection process is painless, takes only a few minutes, and does not affect your birth experience.

Cephalic presentation means your baby is lying head-down in the uterus, which is the ideal and most common position for birth. The baby's head is closest to your cervix and will lead the way through the birth canal during labour. Most babies naturally settle into this position by 32 to 36 weeks of pregnancy as the heaviest part of their body, the head, gravitates downward. Cephalic presentation gives you the best chance of a straightforward vaginal delivery. Your midwife or doctor will confirm the baby's position at antenatal appointments through abdominal palpation and, if needed, an ultrasound scan.

Colostrum is the thick, yellowish milk your breasts produce from about the third trimester through the first two to five days after birth. It comes in small amounts, typically just a few millilitres per feed, but that is exactly what a newborn's tiny stomach needs. Colostrum is packed with antibodies, white blood cells, and growth factors that protect your baby from infections and help seal their gut lining. It also acts as a gentle laxative, helping your baby pass meconium and reducing the risk of jaundice. Every drop counts.

Cluster feeding is when your newborn wants to feed very frequently, sometimes every 30 to 60 minutes, in a concentrated stretch of a few hours, usually in the late afternoon or evening. This is normal newborn behavior driven by hunger, growth spurts, or a natural need for comfort and closeness. Cluster feeding also signals your body to produce more milk, so it serves an important supply-building purpose. It can feel exhausting and may make new parents worry that their milk is insufficient, but a baby who is producing enough wet and dirty nappies and gaining weight is usually feeding well. Try to rest between feeds and accept offers of help.

Combination feeding means giving your baby both breast milk and infant formula, either at different feeds or within the same feed. Some parents start combination feeding from birth due to low supply or a NICU stay, while others introduce formula gradually when returning to work or as a way to share feeding duties with a partner. It can help extend your breastfeeding journey rather than ending it completely. To protect your milk supply, try to replace breastfeeds with pumping sessions when you substitute a formula feed. Combination feeding is a flexible approach and there is no single right way to do it.

A dating ultrasound is a scan performed early in pregnancy, usually between 8 and 13 weeks, to confirm or establish how far along you are and to calculate your estimated due date (EDD). The sonographer measures the embryo or fetus and compares this to standard growth charts. If the scan date differs from your LMP calculation by more than a few days, the scan result usually takes priority because it is more reliable. A dating scan also checks for a heartbeat, the number of embryos, and the location of the pregnancy in the uterus.

A doula is a trained non-medical support person who provides continuous emotional, physical, and informational support before, during, and after birth. During labor a doula might offer breathing reminders, massage, position suggestions, and calm reassurance. Unlike midwives or obstetricians, doulas do not perform medical tasks, but research consistently shows that continuous doula support is linked to shorter labors, lower rates of cesarean section, and greater birth satisfaction. Postpartum doulas can help at home with newborn care, infant feeding, and parental recovery. Hiring a doula is a personal choice that works alongside, not instead of, your medical team.

DHA, or docosahexaenoic acid, is an omega-3 fatty acid that is essential for the development of your baby's brain, eyes, and nervous system, especially during the third trimester and the first months of life. Your body cannot make enough DHA on its own, so you need to get it from food or supplements. Oily fish such as salmon and sardines are the richest dietary sources, but many pregnant women in Singapore do not eat enough to meet recommendations. Most prenatal vitamins include 200 to 300 mg of DHA, which is the amount supported by international guidelines.

Dilation refers to the opening of the cervix as your body prepares for and progresses through labor. It is measured in centimeters, starting at 0 (fully closed) and reaching 10 centimeters, which signals that your cervix is fully open and you are ready to push. Early labor typically brings dilation from 0 to 6 cm, while active labor takes you from 6 to 10 cm. Your midwife or obstetrician checks dilation through an internal examination. Progress can feel slow at first, but it often speeds up in active labor. Dilation alone does not determine when you will deliver, as effacement and the baby's position also matter.

Delayed cord clamping means waiting at least 60 seconds, and often one to three minutes, after birth before clamping and cutting the umbilical cord. During this time, blood continues to flow from the placenta to your newborn, transferring extra red blood cells and iron. This boost can improve iron stores for the first several months of life, which is particularly important for babies at risk of iron deficiency. The World Health Organization recommends delayed clamping for all healthy births. It is safe for both vaginal and caesarean deliveries unless the baby needs immediate resuscitation or the mother is bleeding heavily.

Diastasis recti is a separation of the two parallel bands of abdominal muscle (the rectus abdominis) that run down the centre of your belly. As your uterus expands during pregnancy, the connective tissue between these muscles stretches, creating a gap. After birth, this gap may remain, causing a visible ridge or bulge down the midline when you sit up, a feeling of weakness in your core, or lower back pain. Mild cases often improve on their own with guided exercise, but certain movements like sit-ups can make the gap wider if done too soon. Staying well hydrated, attending all antenatal appointments, and reporting symptoms like severe headaches or swelling promptly can help your care team catch problems early.

The umbilical cord is a flexible, rope-like structure roughly 50 to 60 cm long that connects your baby's belly button to the placenta. It contains two arteries and one vein, all cushioned by a jelly-like substance called Wharton's jelly. The vein carries oxygen-rich blood and nutrients to your baby, while the arteries return waste products to the placenta for disposal. After birth, the cord is clamped and cut, leaving a short stump that dries up and falls off within one to three weeks. Delayed cord clamping, waiting at least 60 seconds before cutting, is now recommended by the World Health Organization because it boosts the baby's iron stores.

Uterine involution is the process by which your uterus shrinks back toward its pre-pregnancy size after you give birth. Right after delivery, your uterus weighs about 1 kilogram and sits at your belly button level. Over the next six weeks, it gradually contracts and returns to roughly 60 grams. This process is driven by hormones, particularly oxytocin, which is released naturally and also when you breastfeed. You may feel your uterus as a firm ball in your lower abdomen in the first days. If involution is slow, your doctor may recommend uterine massage or medication to help it along.

Uterine atony happens when the uterus fails to contract firmly after a baby is delivered. Normally, the uterus squeezes down tightly after birth to compress the blood vessels where the placenta was attached. When it stays relaxed or floppy instead, those vessels bleed freely, leading to heavy blood loss. It is the most common cause of postpartum haemorrhage (PPH). Risk factors include a long labour, a large baby, twins, or having had many previous births. Midwives and doctors treat it with uterine massage, oxytocin, and other medications to help the uterus clamp down and stop bleeding.

Certain vaccines are safe and recommended during pregnancy because they protect both you and your newborn. When you are vaccinated, antibodies pass through the placenta to your baby, giving them protection in the weeks after birth before they are old enough for their own immunisations. In Singapore, the influenza vaccine and the Tdap vaccine are the two most commonly recommended vaccines during pregnancy. Live vaccines such as MMR and chickenpox are not safe during pregnancy and should be given before conception or after birth. VBAC stands for vaginal birth after cesarean. It means giving birth vaginally in a pregnancy that follows a previous C-section. Many women are good candidates for VBAC, and a successful VBAC avoids the longer recovery time that comes with repeat surgery. The main risk is uterine rupture at the previous scar, which is uncommon but serious. Your doctor will review your surgical history, the type of uterine incision, your baby's position, and how your labor progresses before recommending VBAC. Most hospitals in Singapore support VBAC with continuous fetal monitoring throughout labor.

Vacuum extraction is an assisted delivery method where a doctor or midwife attaches a soft silicone suction cup to your baby's head during the pushing stage of labour. Gentle traction is applied with each contraction to help guide the baby out. It is used when labour is not progressing, when the baby shows signs of distress, or when the mother is too exhausted to push effectively. The procedure is generally safe, but your baby may have a temporary swelling or bruise on the scalp called a chignon, which fades within a few days.

Vernix caseosa is a white, waxy coating that forms on a baby's skin inside the womb, usually from around 20 weeks of pregnancy. It is made of shed skin cells, water, and oils, and acts as a natural moisturiser and protective barrier. At birth, some babies are covered in it, while others have very little, especially if they are born past their due date. Many hospitals now recommend leaving vernix on the skin for a short time after birth, as it helps retain moisture and may support the newborn's immune defences.

A very preterm baby is born before 32 completed weeks of pregnancy. At this stage, nearly every organ system is immature, so these babies almost always need care in a Neonatal Intensive Care Unit (NICU). Common challenges include respiratory distress syndrome, difficulty regulating body temperature, feeding intolerance, and a higher risk of infection. Brain development is also vulnerable, so doctors will monitor for complications such as intraventricular hemorrhage. Parents play a vital role through skin-to-skin (kangaroo) care, which supports bonding and stability even in the NICU setting.

Water breaking is the common term for rupture of membranes, when the amniotic sac that surrounds your baby tears and releases amniotic fluid. This can feel like a sudden gush or a slow, steady trickle. It may happen on its own during active labor, or your doctor may break it deliberately to speed things along. If your membranes rupture before contractions begin, labor usually follows within hours. You should note the time, color, and smell of the fluid. Clear or pale yellow fluid is normal, but green or brown fluid may signal meconium and requires immediate medical attention.

A water birth involves laboring in a warm-water pool and, if all goes smoothly, delivering your baby while still in the water. The warmth relaxes muscles, eases contraction pain, and supports your weight so you can move into comfortable positions. Babies transition gently from the amniotic fluid environment and take their first breath only once lifted to the surface. Water births require continuous monitoring and a midwife present throughout. They are only suitable for low-risk pregnancies with no complications. If any concerns arise, you will be asked to leave the pool promptly for safer management.

Water immersion means spending part of your active labor in a warm bath or birthing pool without necessarily giving birth in the water. The buoyancy reduces the pressure on your joints and pelvis, while the warmth relaxes tense muscles and can slow the release of stress hormones, making contractions feel more manageable. Studies show water immersion in the first stage of labor reduces the need for epidurals and shortens labor duration for some women. You can enter the water once active labor is established, usually around 5 to 6 centimeters dilated. You are free to leave the pool at any time and switch to other pain relief.

The yolk sac is a small, round structure visible on ultrasound from about 5 to 6 weeks of pregnancy. Before the placenta is fully formed, it supplies nutrients and oxygen to the developing embryo and plays a key role in early blood cell production. A visible yolk sac on a transvaginal scan is a reassuring sign that the pregnancy is progressing inside the uterus, not in the fallopian tube. Its size and shape give sonographers useful clues about how the embryo is developing. An unusually large, small, or irregular yolk sac may prompt closer monitoring in the weeks that follow.
