Fever in Babies and Children: When to Act
Fever is defined as a rectal temperature above 38.0°C (100.4°F). Axillary (armpit) readings run ~0.5°C lower.

| Under 3 months | Any rectal temp 38.0°C+ | Go to A&E immediately - Fever at this age is a medical emergency |
| 3-6 months | 38.0°C+ with other symptoms | Call your paediatrician same day; monitor closely |
| 6-24 months | 38.0°C+ lasting >24h | See doctor if no clear cause; immediately if rash, breathing issues |
| 2-5 years | 39.0°C+ or lasting >3 days | See doctor; give paracetamol/ibuprofen for comfort |
| Over 5 years | 40.0°C+ or lasting >5 days | See doctor; manage comfort with medication |
How to Take Temperature Accurately
Rectal
Under 3 months
Most accurate; use a rectal-specific thermometer with lubrication
Axillary (armpit)
Any age for screening
Add 0.5°C for true reading; less accurate than rectal
Temporal / ear
Over 6 months
Convenient; must read in correct ear canal angle
Call 995 or go to A&E immediately if: baby under 3 months has any fever; child has difficulty breathing, purple/red rash, seizure, stiff neck, is unresponsive, or fever above 41°C.
