How Long Does It Take to Get Pregnant?

Evidence-based answer with age-specific conception rates, factors that speed or slow things down, and guidance on when to seek help in Singapore.

The Short Answer

For couples having regular unprotected sex, about 85% will conceive within 12 months. About 92-95% conceive within 2 years. The average for couples under 35 is 4-6 months of trying.

But that average hides a wide range. Some couples conceive on the first cycle. Others, with no underlying fertility problem, take 10-11 months. Both are completely within the range of normal. The randomness of when conception happens is not a sign of anything being wrong.

Why it feels so unpredictable

Even under perfect conditions - healthy couple, perfectly timed sex during the fertile window - the chance of conception per cycle is only about 20-25% for a woman under 30. This is not a fertility problem. It reflects the biology of human reproduction, which is far less efficient than most people assume.

Conception Rates by Age

Age is the single biggest factor affecting how quickly you conceive. Egg quality and quantity decline steadily from the mid-30s, with a sharper drop after 37.

Singapore's average age at first birth was 31.9 years in 2023, meaning many local couples are starting in the age bracket where timing and efficiency really matter.

Factors That Affect How Long It Takes

Age is the most important factor, but several others can speed up or slow down conception:

Things That Help

  • - Accurate ovulation tracking (OPK + mucus monitoring)
  • - Regular sex every 1-2 days in the fertile window
  • - Healthy BMI (18.5-24.9 for women)
  • - No smoking for both partners
  • - Starting folic acid 1-3 months before trying
  • - Limiting alcohol to zero or near-zero

Things That Slow It Down

  • - Irregular or anovulatory cycles (PCOS, thyroid)
  • - Poorly timed sex (missing the fertile window)
  • - Underweight or overweight BMI
  • - Male factor issues (low count, poor motility)
  • - Blocked tubes, endometriosis, fibroids
  • - High stress (affects cycle regularity and ovulation)

About 30% of infertility is due to female factors, 30% to male factors, and 30-40% to combined or unexplained causes. This is why both partners should be investigated if conception is taking longer than expected.

When to See a Doctor in Singapore

Most couples benefit from a basic fertility check even before hitting the standard waiting periods, especially if there are known risk factors. A GP visit costs $20-50 at a polyclinic and can flag obvious issues early.

See a doctor after 12 months (under 35)

This is the international guideline. Your GP will arrange a semen analysis and blood tests for ovarian reserve (AMH) and hormone levels. If tests are normal, you may be referred for monitoring cycles.

See a doctor after 6 months (age 35-37)

Do not wait a full year. Ovarian reserve declines quickly in the late 30s and earlier investigation gives you more options.

See a doctor immediately (over 38, or known issues)

If you are over 38, have irregular periods, suspected PCOS or endometriosis, have had previous pelvic infections, or your partner has had a previous fertility test showing concerns - do not wait.

Maximising Your Chances Each Cycle

Most couples who take a long time to conceive are simply missing their fertile window. Having sex two or three times per week, every week, means you will rarely miss ovulation. For couples who want to be more targeted, using an ovulation predictor kit (OPK) to identify the 2-3 days before ovulation is the single most effective change you can make.

The Ovulation Calculator gives you an estimated fertile window based on your cycle length. This is a good starting point. Combine it with OPK testing for the best accuracy. OPK strips are available at Guardian, Watson, and Cold Storage for under $20 per cycle.

If you have been tracking for a few cycles and consistently have no ovulation signal on OPK tests, or your cycles are irregular (shorter than 21 days or longer than 35), speak to a GP. This could indicate PCOS, thyroid issues, or other treatable conditions.
