Why Folic Acid Matters So Much

Folic acid (vitamin B9) is essential for the development of the neural tube - the structure that forms the baby's brain and spinal cord. The neural tube closes by 28 days after conception - often before a woman even knows she is pregnant.

Inadequate folic acid during this critical window increases the risk of neural tube defects (NTDs), including spina bifida and anencephaly. These are serious, often disabling or fatal conditions that are largely preventable.

Taking folic acid before conception and through the first trimester reduces the risk of NTDs by 50-70%. This is why the recommendation is to start before you try to conceive, not after you get a positive test - by then, the neural tube is already closing.

Singapore rates and guidelines

Neural tube defects in Singapore occur in approximately 1-2 per 1,000 live births. KKH, NUH, and HPB all recommend 400mcg folic acid daily for all women planning pregnancy, started at least 1 month before conception and continued through the first 12 weeks of pregnancy.

For most women, a standalone 400mcg folic acid supplement is sufficient and cost-effective. A comprehensive prenatal vitamin is also fine and provides additional nutrients (iron, iodine, vitamin D, DHA) that support pregnancy from the start.

Other Supplements Worth Considering

Vitamin D - check yours first

Despite Singapore's sunshine, vitamin D deficiency is common (indoor jobs, sunscreen use). Deficiency is associated with reduced fertility and pregnancy complications. Get tested ($20-40 at GP), and supplement if below 75 nmol/L.

Iodine 150mcg/day

Iodine is essential for fetal thyroid development and brain development. Most prenatal vitamins include it. If using a standalone folic acid, consider adding an iodine supplement.

DHA (omega-3) 200mg/day

Supports fetal brain and eye development. Can be from fish oil capsules or algae-based DHA (suitable for vegetarians). Begin preconception; safe throughout pregnancy.

Iron - only if deficient

Iron is included in most prenatal vitamins. Do not supplement iron unless tested and found to be deficient - excess iron causes constipation and can be harmful. Test ferritin with your GP.
